Diabetes Care for Older Adults at Home

How home care supports diabetes management in older adults, and prevents the complications that lead to hospitalisation.

Diabetes is one of the most common conditions among older adults, and one of the most preventably-complicated. Daily home care can dramatically reduce complications and emergency department visits.

Blood sugar monitoring

Older adults often have unpredictable blood sugar patterns. Caregivers conduct scheduled testing, log results, and watch for hypoglycemia (low blood sugar), which is a major fall risk in older adults.

Insulin administration

Many older adults need insulin but cannot reliably self-administer. Trained caregivers can prompt or assist depending on care plan and state regulations. Coordination with the endocrinologist or primary care is constant.

Nutrition

Diabetic-friendly meals must be appealing or the patient will not eat them. We adapt cuisine to family preferences while keeping carbohydrate timing and portion control consistent.

Foot care and wound prevention

Diabetic neuropathy means foot injuries can go unnoticed and become serious. Caregivers conduct daily foot inspections and report concerns immediately.

Complication awareness

We watch for signs of progression, vision changes, kidney function changes, neuropathy worsening, and cardiovascular symptoms. Each warrants prompt clinical follow-up.
