24-Hour Home Care Cost in Houston (2026 Breakdown)

Families ask this question in almost every first call: "How much does 24-hour home care actually cost in Houston?" Here are the 2026 numbers, without gates or lead forms.
Two formats, two price points
24-hour rotating puts 2 or 3 caregivers in continuous awake shifts (typically 8 or 12 hours each). Nobody sleeps on the job. It's used for higher-acuity cases: wandering, delirium risk, post-surgical monitoring, or clients who need constant supervision. Houston rate: $650 to $850 per day ($19,500 to $25,500 per month).
Live-in puts one caregiver in the home 24 hours with an 8-hour uninterrupted sleep window and meal/rest breaks during the day. Used for lower-acuity but still-full-time cases: a client who sleeps through the night and needs company/help during the day. Houston rate: $450 to $650 per day ($13,500 to $19,500 per month).
The 30% cost difference is real. So is the difference in what each format can safely cover. Our [Care Manager assessment](/services/24-hour-live-in-care/) walks through which fits your loved one.
What drives the variation in each format
Three variables move the rate:
- Acuity. Dementia with wandering, post-stroke with swallowing precautions, or ventilator-dependent all push toward the upper end.
- Location. Client in [River Oaks](/services/24-hour-live-in-care/river-oaks/) or a large [Memorial estate](/services/24-hour-live-in-care/memorial/) may need caregivers who are comfortable in staffed households; that's a match constraint that trends higher.
- Continuity requirements. Requesting the same 2 caregivers on a permanent rotation (recommended for dementia) is more expensive than accepting scheduling flexibility.
The funding math that almost every Houston family misses
For a client at $700/day (24-hour rotating), that's $21,000/month. Nobody funds that from cash flow. Here's what actually pays:
- LTCI: A representative $200/day policy pays $6,000/month once the elimination period is served, offsetting almost 30% of the cost. Send us the policy for a [free review](/paying-for-care/long-term-care-insurance/).
- VA Aid & Attendance: Up to $2,795/month for eligible veterans (2026). Not a huge fraction of 24-hour cost, but real. [Eligibility screener here](/paying-for-care/va-aid-and-attendance/).
- Home-equity line: Many Houston families with significant home equity use a HELOC to fund 24-hour care rather than sell the home. Talk to a fee-only planner.
- Private-pay from savings: The reality for the balance. We invoice weekly, no long-term contract, adjust the schedule any time.
What to ask any Houston agency quoting 24-hour care
1. Is your caregiver a W-2 employee or a 1099 contractor? *(W-2 keeps workers' comp and background-check accountability on the agency.)*
2. What's your fill-rate if a scheduled caregiver calls out? *(A 24-hour case can't have a gap.)*
3. Do you require a written care plan reviewed by a Registered Nurse? *(For 24-hour, RN oversight is a floor, not a nice-to-have.)*
4. Do you bill LTCI directly or do we front the cost?
The bottom line
24-hour home care in Houston is a serious financial commitment and often the right one — cheaper than most Houston memory care facilities for the same acuity, and lets the client age in the home they know. Our [interactive cost estimator](/cost-estimator/) shows the monthly range for your specific scenario, and a [free Care Manager visit](/contact/) gives you an itemized quote before any commitment.
