How to Hire a Home Caregiver in Houston — Agency vs. Private Hire, Explained

Every Houston family considering home care faces this decision: hire through an agency, or hire a caregiver directly. Both work. But the tradeoffs are meaningful and often mis-understood.
The three real differences
Cost per hour is the visible difference. The three that actually matter over time:
1. Employment status. An agency caregiver is a W-2 employee of the agency. A private-hire caregiver is either your household employee (with tax withholding, workers' comp, and unemployment insurance obligations sitting on you) or a 1099 independent contractor (which the IRS will almost always reclassify as a household employee if audited). Getting this wrong creates real tax and legal exposure for the family.
2. Backup and continuity. When your private-hire caregiver has the flu, is sick, has a family emergency, or resigns, you're providing the care yourself until you find someone new. An agency has a bench and fills the shift.
3. Supervision and clinical oversight. A private-hire caregiver reports to you. Any judgment call about medication observation, sundowning, wound care, or symptom escalation is on you to make. A well-run agency has nurse supervision, so a Care Manager reviews the care plan, sets the observation protocols, and takes escalation calls.
When private hire makes sense
- Very stable, low-acuity situations where the same person can consistently show up for months without variation
- Neighbor or family friend who's already trusted, has time, and is being paid modestly for what they'd help with anyway
- Highest cost sensitivity where the family accepts backup coverage as their own problem
Even then: run background checks, verify references, buy household employer insurance, and pay through a payroll service (Care.com HomePay, GTM Payroll, or Poppins Payroll) so tax withholding is handled correctly.
When an agency is worth the premium
- Anything higher-acuity — dementia, post-stroke, post-surgical recovery, LTCI-billed care
- Anything 24-hour — the operational complexity of covering shifts, holidays, sick days, and vacations is real
- Any situation where you can't be the backup — you work full-time, live out of town, or have young kids at home
- Any nurse-supervised requirement — LTCI carriers, workers' comp, and some Medicare Advantage plans require nurse-supervised care
What to look for in a Houston agency
1. W-2 caregivers, not 1099. This is the single fastest disqualifier. If the agency claims their caregivers are "independent contractors," they're either misclassifying or exposing you to liability. Ask directly.
2. Nurse-supervised care plans. A Registered Nurse should review the plan, do the initial assessment, and be on call for escalations. Many Houston agencies don't have a nurse on staff — ask.
3. Owner or Care Manager access. Who picks up the phone at 9 p.m. on Sunday? A call center is a red flag. A local Care Manager, or the owner, is the standard.
4. HHSC licensure. Texas requires Personal Assistance Services (PAS) licensure. Every legitimate Houston home-care agency has this. Ask for the license number and look it up.
5. Insurance certificate. Ask for proof of general liability, professional liability, and workers' compensation coverage. If they won't share it, walk away.
6. Written care plan and itemized invoice. Both should be provided before any care starts. Verbal quotes are a red flag.
