Paying for care · Houston

Does Medicare cover home care in Houston?

Short answer: no. Traditional Medicare does not pay for the kind of ongoing, non-medical home care most Houston families are searching for — help with bathing, dressing, meals, medication reminders, companionship, dementia supervision, 24-hour cover. Medicare pays for short, prescribed episodes of skilled medical care (home health) after a qualifying event, not for the everyday help that lets an older adult stay at home safely. What Medicare will not cover, long-term care insurance often will, VA Aid & Attendance covers for eligible veterans, and private pay covers the rest. We walk every family through this on the first call, in plain English.

Eligibility

  • Traditional Medicare Part A / Part B does NOT cover long-term custodial home care in Houston or anywhere else
  • Medicare Part A does cover short-term home HEALTH (skilled nursing, physical therapy, occupational therapy) for up to 60-day episodes if you are homebound AND under a physician's plan of care AND need intermittent skilled care
  • Medicare Advantage (Part C) plans MAY offer limited custodial home-care benefits — coverage varies by carrier and plan year; we help you check
  • Medicare does NOT cover: bathing help, meal prep, companion visits, 24-hour or live-in care, dementia supervision, medication reminders, transportation
  • Dual-eligible Medicare + Medicaid (Texas STAR+PLUS) enrollees may qualify for additional in-home services through Medicaid; separate program

What it covers

  • Home HEALTH (not home care): part-time or intermittent skilled nursing, physical therapy, occupational therapy, speech-language pathology, medical social services, home health aide (only if receiving one of the skilled services above)
  • The home-health aide portion under Medicare is short-term, tied to the skilled-care episode, and ends when the episode ends — usually days or a few weeks, never open-ended
  • Some Medicare Advantage plans (varies by year and carrier) cover a small allowance for in-home custodial services — often $100–$500 quarterly, or a handful of hours
  • Durable medical equipment (walker, hospital bed, wheelchair) with a physician order

Common questions

Does Medicare cover home care?

No, not the kind of ongoing daily-living help (bathing, dressing, meals, companionship, dementia supervision, 24-hour cover) most Houston families are looking for. Traditional Medicare only covers short episodes of skilled home HEALTH — nurse or therapy visits after a qualifying event, for a limited number of days, and always tied to a physician's plan of care. What Medicare will not cover, long-term care insurance often will.

What is the difference between home HEALTH and home CARE?

Home HEALTH is skilled medical care delivered at home by a nurse, physical therapist, occupational therapist or speech therapist under a physician's order. It is short-term, episode-based, and Medicare pays. Home CARE is non-medical help with the activities of daily living — bathing, dressing, meals, medication reminders, mobility, companionship. It is long-term, hour-based, and Medicare does NOT pay. Families frequently need both at the same time; we coordinate with the home-health agency so nothing falls through the cracks.

Does Medicare Advantage cover home care in Houston?

Sometimes, in small amounts, and it varies by carrier and plan year. Since 2019 CMS has allowed Medicare Advantage plans to add supplemental in-home custodial benefits, and some Houston-area plans do — often a modest hours-per-quarter allowance or a fixed dollar benefit. Bring your MA plan's Evidence of Coverage document to our free consultation and we will read it with you.

My parent is being discharged from Houston Methodist. Will Medicare pay for a caregiver at home?

For the first 60 days after a qualifying hospital stay, Medicare will pay for a Medicare-certified home HEALTH agency to send a nurse, therapist, and (if you receive one of those) a home-health aide for short, intermittent visits. That is not the same as a caregiver who stays with your parent for the day. Most families layer non-Medicare home care alongside the home-health episode so someone is actually present during the risky first two weeks; we do exactly this coordination routinely.

If Medicare will not pay, what does?

In order of most-to-least common for our Houston families: (1) Long-term care insurance if a parent bought a policy — most policies were designed for exactly this, and we bill the carrier directly; (2) VA Aid & Attendance for eligible wartime veterans and surviving spouses — up to about $2,700/month tax-free; (3) Private pay from the household budget or a home-equity or reverse-mortgage arrangement; (4) Medicaid STAR+PLUS for dual-eligible Medicare + Medicaid enrollees in Texas — separate program with its own eligibility. We walk you through each on the free consultation call.

Is there any way to get Medicare to cover long-term home care?

No. This is one of the most misunderstood parts of the entire senior-care system in the United States. Families frequently arrive at retirement believing Medicare will cover long-term care and are shocked to learn it does not. The two federal programs that do cover long-term custodial care are Medicaid (for those who qualify financially) and, on a limited basis, VA Aid & Attendance (for eligible veterans). Everything else is private pay, long-term care insurance, or family caregiving.

The bottom line

Traditional Medicare does not cover ongoing home care in Houston or anywhere else. It covers short episodes of skilled home HEALTH after qualifying events, and some Medicare Advantage plans include small custodial benefits. For the daily-living help most families need, long-term care insurance, VA Aid & Attendance, private pay, or Medicaid STAR+PLUS are the actual funding sources. A free 15-minute call with our team tells you which one applies to your parent — no pressure, plain English.
